  If you are a current DJP student you do not need to resubmit these forms for summer camp.
  Please confirm with the office that all your documents are up to date.

1. Copy of child’s Birth Certificate
2. Copy of parents’ Government Issued Photo ID
3. Current Medical Forms / Statement of Good Health
4. Current Immunization Records for child
