Kindly submit the following to complete the registration process.
If you are a current student at DJP, your school forms, other than the camp registration form, can be used for camp.
Please contact the office to make sure they are all up-to-date.

- $100.00 non-refundable Registration Fee per child (on camp payment form HERE)
- $250.00 non- refundable fee for Security (on camp payment form HERE)
- Copy of child’s Birth Certificate
- Copy of parents’ Government Issued Photo ID
- Current Medical Forms / Statement of Good Health
- Current Immunization Records for child

All documents that need to be signed and returned can either be:

3. Uploaded to the registration forms online
